1. Information we collect
We collect only what we need to treat you safely and to administer your appointments and billing:
- Contact details — name, date of birth, phone, email, address and an emergency contact.
- Health information — your reason for visiting, relevant history, medications, assessment findings and treatment notes.
- Coverage details — insurer, policy or member number, and for a motor vehicle accident your ICBC claim number and date of accident.
- Website and booking data — what you send through our forms, plus basic analytics about how the site is used. [Confirm which analytics tools are in use.]
2. How we use it
Your information is used to assess and treat you, to keep the clinical records the College requires, to book and confirm appointments, and to bill your extended health plan or ICBC where you have asked us to.
We do not sell your information, and we do not use your health information for marketing. If we ever send clinic updates by email, they will be optional and you can unsubscribe at any time.
4. Storage, security and retention
Records are kept in a password-protected clinical records system with access limited to the treating therapists and clinic administration. Paper forms are stored in a locked cabinet on site and shredded once entered.
We keep clinical records for the period required by the College and then destroy them securely. [Confirm the current retention period, including the rule for patients treated as minors.]
5. Your rights
You can ask to see your records, ask us to correct anything inaccurate, withdraw consent for a use you previously agreed to, or ask how your information has been handled. Consent to treatment can also be modified or withdrawn at any point during a session.

Our forms send what you type to the clinic so we can reply. Please do not include detailed health information in a general enquiry — the intake form is the secure place for that.
